Help shape the end-of-life care in Hammersmith & Fulham
The NHS and its partners in North West London want to know more about the care and support delivered by their community-based specialist palliative care services (or End of Life care) for adults (18+).
By Community-based Specialist Palliative Care (CSPC), they are a referring to specialist level of palliative care that is delivered in a setting that is not within a hospital or a GP surgery, but rather in a patient’s own home, a care home, a hospice and a community hospital or centre.
Examples of these types of care services include – hospice bedded inpatient care, Hospice day and outpatient services, care at a Nursing Home, community specialist palliative care nursing team providing care and support in the home and Hospice@home service.
They want to know what is important to you and what needs to improve to ensure they deliver high quality care and excellent patient and family/ carer experience through these services.

This event is especially aimed at local people from Hammersmith and Fulham as they want make sure their voice is heard as they look to develop services for adults that meet all of our diverse communities needs and provide high quality care and good patient experience when they are on their journey to the end of their life. Your feedback will play an important part in helping them make sure their services meet your needs.
