As discussed at the Forum, there will be a Chair election at the January Forum on 18th January 2019.
You can download an application form to complete and return should you wish to apply for the Role here.
You will see the Role description and Person Specification for the Chair role in the Terms of Reference for Forum which can be downloaded here.
Should you wish to apply, please do so by the end of the day on Friday 30th November 2018 by any of the following delivery methods:

- In person by coming to our office at Carers Network, Beethoven Centre, Third Avenue, W10 4JL
What happens after 30th November?
We will send all received applications to all carers by the end of the following working day, Monday 3rd December so that all those who are attending the next Forum can cast their vote on who they would like elected as Chair.
For those who are not able to attend the next meeting, but wish to vote, please do so by emailing Sonia Benitez by the end of the day on Thursday 17th January 2019 so that your vote can be counted.
Those who are putting themselves forward as Chair will not be able to take part in the voting process.
A formal agenda for the meeting on Friday 18th January 2019 will be sent prior to the meeting.
